    Job Description:

    Under the supervision of a Registered Nurse, coordinates the Plan of Care for the patient in the home, hospital or other setting. Works cooperatively with other team members in maintaining standards of professional nursing practice and in accordance with age specific needs and within the scope of practice for License Practical Nurse within New York State.

    Minimum or Preferred Qualifications:

    Associate of Applied Science (AAS) in Licensed Practical Nursing is required. 1 year of experience in Acute Medical/Surgical nursing is preferred. Valid NYS License as a Practical Nurse (LPN) upon hire is required. Valid NYS Drivers License upon hire is required.

    Salary Range:

    $23.98-$31.59 per hour Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.
