  • Health Advocates Network is currently seeking a Licensed Master Social Worker 2 to work at a facility in New Hampton, New York. These are registry positions with our company.

    Pay Rate: $63 / hour

    Shift details:

    • M-F 8:00 AM - 4:30 PM

    Duties included but not limited to:

    • Completion of, and maintaining, medical record documentation within required designated time-frames.

    • Participate in assigned meetings: Treatment plan meetings (including 730/330 rounds), Daily morning rounds, Therapeutic Community Meetings, Case Reviews, Incident debriefings in the absence of the Program Coordinator/Treatment team leader

    • Facilitate clinical groups during the day, as well as Provide individual therapy to assigned patients

    • Secure and maintain the Record of Arrest and Prosecution information

    • Secure collateral information from various sources

    • Maintain ongoing contact with patient's family and significant others, as assist families in finding support services and engage families in the treatment process

    • Secure approval for children visitation, special visitation requests and notify necessary staff. Coordinate electronic visitation and family participation in treatment team meetings

    • Participates in the facility's annual Family Day, quarterly Family Educational meetings, Annual CORE curriculum training and in-service programming

    • Assist patients with case management tasks, including Coordinate the Advance Directives process (e.g., Health Care Proxy)

    • Senior Social Work staff train and supervise entry level Social Work staff and student interns

    • Participates on various hospital committees
