Post-assessment, the IHT provides brief, evidence-based therapy, and coordinates referrals, where appropriate. The IHT comanages patient care along the PCP and acts as the main contact for and liaison between the patient's referring provider, psychiatry team, care coordination team, and other treatment team members. The IHT supports medical providers through ongoing consultation and provides continuous mental and behavioral health education. Patient care is provided both in person and via telehealth depending on patient preferences and scheduling availability.
